Filing 4 Order and Notice of Deficiency: Arthur Cross, Jr must address the following deficiency: Plaintiff has failed to provide the in-forma-pauperis motion and inmate trust account statement. The Court notes that to the extent Plaintiff seeks to maintain a new civil lawsuit challenging the conditions of his confinement, or facts arising from his confinement, he has not presented this suit on the completed form as required by the miscellaneous orders of this Court. Thus, he has failed to provide answers to important questions, and to acknowledge sanctions warnings that are included in the form. If Plaintiff wishes to maintain this suit, he must correct this deficiency by providing a properly completed complaint form. It is further ORDERED that, if Plaintiff wishes to maintain this lawsuit, he shall cure each aforementioned deficiency within thirty (30) days of the date of this order. Failure to timely comply with this order could result in the dismissal without prejudice of Plaintiff's complaint without further notice for failure to prosecute pursuant to Federal Rule of Civil Procedure 41(b).
