October 31, 2022 Opinion or Order Filing 6 SECOND ORDER AND NOTICE OF DEFICIENCY: Accordingly, it is ORDERED that Plaintiff shall have fourteen (14) days from the date of this Order to file an amended IFP application using form AO 239, Application to Proceed In Forma Pauperis (Long Form). As required by the Court's October 3 order, Plaintiff's amended IFP application must address the following deficiencies: 1. Plaintiff shall sign and date the application. 2. To the extent Plaintiff continues to attest that his monthly income is $0 (other than nominal gifts), he must either: a. Identify who pays for his living expenses or b. If no one pays for his monthly expenses, Plaintiff must explain why he does not pay for monthly living expenses and substantiate that assertion with supporting documents. 3. Plaintiff must identify whether he is currently employed. This is Plaintiff's final opportunity to supply the requisite financial information. (Ordered by Magistrate Judge D.
