Filing 10 ORDER: Plaintiff's application for leave to proceed in forma pauperis #8 is granted. The Court orders the trust fund officer at Plaintiff's place of incarceration to immediately deduct $4.36 from Plaintiff''saccount for payment to the Clerk of Court as an initial partial payment of the filing fee and to continue making monthly deductions in accordance with this order (100% of Plaintiff's income - this includes gifts, prison payroll, settlements, etc. - when it meets statutory minimums, must be devoted toward his filing fees). The Court directs the Clerk of Court to (1) send a copy of this order to the trust fund officer at Dixon Correctional Center and to the Court's Fiscal Department. However, summonses shall not issue. Plaintiff's motion showing good cause why the court should allow his time barred complaint to go forward #3 is denied. Plaintiff's complaint #1 is dismissed for failure to state a claim because it is time barred on its face. In addition, the Court finds that the complaint is frivolous in light of the obvious and conceded time bar. This dismissal counts as a dismissal under 28 U.S.C. 1915(g). Plaintiff's "motion and notice to the court" #6 is denied as the motion fails to seek any necessary relief from the Court (of course, Plaintiff cannot provide case numbers that he does not have). Filing 7 ORDER: Plaintiff's application for leave to proceed in forma pauperis #5 is denied without prejudice. If Plaintiff wants to proceed with this lawsuit, he must either (1) submit a renewed application that is certified by an appropriate official and accompanied by a copy of Plaintiff's trust fund statement showing all transactions in his account for the six-month period immediately preceding his initiation of this action and include an accurate accounting of his income from all sources for the twelve month period for any period of time not incarcerated; or (2) pay the fullstatutory filing fee of $402.00. Failure to comply with this order by June 22, 2023, will result in summary dismissal of this case. The Clerk is directed to mail Plaintiff a blank application to proceed in forma pauperis application and a copy of this order. Plaintiff's "motion for relief" #4 is granted to the extent that Plaintiff need not submit copies of his filings when he submits filings to the Court. Plaintiff is remined that he should keep a copy for himself of all filings. Plaintiff's request for a magistrate consent form is denied without prejudice. Once all Defendants have been served (assuming Plaintiff states a claim) and an appearance has been filed on all Defendants' behalf, Plaintiff should consult with Defendants to determine if all parties wish to consent to the Magistrate Judge for all proceedings and if so, file the completed consent form signed by all parties.
