ORDER: By order dated September 18, 2018, the Court directed Plaintiff to either file an application to proceed in forma pauperis or pay the full $400 filing fee #6 . The Court warned Plaintiff that failure to comply with the order by October 18, 2018, would result in summary dismissal of this case. Plaintiff did not respond to the Courts order. Accordingly, this case is dismissed without prejudice for Plaintiffs failure to comply with the Courts order. The Court directs the Clerk of Court to terminate all pending motions as moot. ORDER:Plaintiff is directed to either file an application to proceed in forma pauperis on the enclosed form or pay the full $400 filing fee. Failure to comply with this order by October 18, 2018, will result in summary dismissal of this case. The Clerk of Court is directed to send Plaintiff a blank application to proceed in forma pauperis and a copy of this order. Plaintiff is instructed to file all future papers concerning this action with the Clerk of the United States District Court for the Northern District of Illinois in care of the Prisoner Correspondent. Plaintiff also must promptly submit a change-of-address notification if he is transferred to another facility or released. If Plaintiff fails to keep the Court informed of his address, this action will be subject to dismissal for failure to comply with a Court order and for failure to prosecute.
