ORDER: Anderson's #12 Motion for Leave to Supplement is DENIED; 2)Anderson's #10 Motion for Leave to Proceed in forma pauperis is GRANTED, required to pay the $350 as set forth below; 3) financial documentation indicates he lacks sufficient income or assets to pay the initial partial filing fee, and payment is therefore DEFERRED; 4) the Clerk shall open an account in Anderson's name and shall complete the Notice of Payment Form (Form EDKY 525) with (a) Anderson's name, (b) his inmate registration number and (c) this case number and shall serve a copy of this Order and the Notice of Payment form upon the Jailer/Warden where Anderson is currently confined and the Office of General Counsel for the Dept of Corrections in Frankfort, KY; 5) Each month Anderson's custodian shall send the clerk a payment in an amount equal to 20% of his income for the preceding month out of his inmate trust fund account, but only if this amount exceeds $10 and shall continue to do so until the entire $350.00 is paid; 6) Anderson must immediately advise the Clerk of any change in his current address.
