Filing 5 Judge Richard G. Stearns: ORDER entered granting #2 Motion for Leave to Proceed in forma pauperis. If Botelho wishes to pursue this action, he must, no later than July 8, 2019, file the necessary prison account statements to cover the period from October 25, 2018 through April 25, 2019. Failure to comply with this directive may result in dismissal of this action. PCCF and BCCF shall be dismissed as defendants in this action. The only claims that will go forward at the present time are those concerning Botelho's access to his medication. Summonses shall issue as to defendants Bethany and Zollo. Botelho is responsible for serving the summonses, complaint, and this order on the defendants in compliance with Rule 4 of the Federal Rules of Civil Procedure and Local Rule 4.1. Botelho must complete the aforesaid service no later than September 1, 2019. Failure to complete service in a timely fashion may result in dismissal of this action without further notice from the court. Because Botelho is proceeding in forma pauperis, he may elect to have service completed by the United States Marshals Service.
