Filing 6 Magistrate Judge Katherine A. Robertson: ORDER entered granting #5 Motion for Leave to Proceed in forma pauperis. The Clerk shall send a copy of this Order to the treasurer of the institution having custody of Plaintiff. The Clerk shall provide Plaintiff with the form for Consent/Refusal of Magistrate Judge Jurisdiction and the instructions for that form. The Clerk shall issue summonses as to all four defendants. Plaintiff is responsible for serving the summonses, complaint, consent package, and this Order on: (1) each of the four named defendants; (2) the United States Attorney for the District of Massachusetts; and (3) the Attorney General of the United States. Because Plaintiff is proceeding in forma pauperis, he may elect to have service completed by the United States Marshals Service Plaintiff shall have 90 days from the date of the issuance of the summons to complete service.
