Plaintiff has asked for leave to proceed in forma pauperis using a two-page form entitled Application to Proceed in District Court Without Prepaying Fees or Costs. On the first page, she indicates that she is unemployed and has not received income from any source in the past twelve months. Except for signing the form, Plaintiff did not complete the second side of the form, in which she is directed to identify the amount of cash she has (in hand or in the bank), other assets of value, monthly expenses, dependents, and outstanding financial obligations. Because Plaintiff did not complete the second page of the form Application, she has failed to provide a complete financial statement, as required by 28 U.S.C. s 1915(a)(1). If Plaintiff would like to proceed with this action, she must, within 21 days, pay the $400 filing fee or file a renewed Motion for Leave to Proceed in Forma Pauperis. If Plaintiff files a renewed Motion for Leave to Proceed in Forma Pauperis, she must respond to all the questions in the form Application. She must also supplement the form Application as necessary to explain how she is able to afford or access the necessities of life. Failure to pay the fee or file a renewed Motion for Leave to Proceed in Forma Pauperis within 21 days may result in dismissal of this action. (PSSA, 3)
