September 5, 2018 Filing 4 Magistrate Judge Katherine A. Robertson: ORDER entered granting 2 Motion for Leave to Proceed in forma pauperis. The Clerk shall provide Plaintiff with the form for Consent/Refusal of Magistrate Judge Jurisdiction and the instructions for that form. The Clerk shall issue a summons as TD Bank Wealth Management, the sole defendant. Plaintiff is responsible for serving the summons, complaint, consent package, and this Order on Defendant in accordance with Rule 4 of the Federal Rules of Civil Procedure. Because Plaintiff is proceeding in forma pauperis, he may elect to have service completed by the United States Marshals Service. Plaintiff shall have 90 days from the date of the issuance of the summons to complete service. Plaintiff is responsible for contacting Defendant (or its attorney) to inquire as to its consent or refusal to proceed before the Magistrate Judge and for ensuring that the Consent/Refusal of Magistrate Judge Jurisdiction form is filed with the Court.
