Hennessy: ELECTRONIC ORDER entered granting #3 Motion for Leave to Proceed in forma pauperis "Plaintiffs Motion for Leave to Proceed in forma pauperis (Docket No. 3) is ALLOWED. Because this action was randomly assigned to this Court pursuant to the District Court's Consent Program, the Clerk shall provide the plaintiff with the form for Consent/Refusal of Magistrate Judge Jurisdiction and the instructions for that form ("consent package"). The Clerk shall issue summons for service of the defendant. The plaintiff may elect to have service made by the United States Marshal Service. If directed by the plaintiff to do so, the United States Marshal shall serve the summons, complaint, consent package and this order upon defendant, in the manner directed by the plaintiff, with all costs of service to be advanced by the United States. Notwithstanding this Order to the United States Marshal Service, it remains plaintiffs responsibility to provide the United States Marshal Service with all necessary paperwork and service information.
