Filing 2 DEFICIENCY ORDER - Because Plaintiff did not provide his prisoner trust account statement or a compliant affidavit in support of his in forma pauperis motion, as required by the Prison Litigation Reform Act of 1995 (PLRA), 28 U.S.C. 1915(a)(h), the Court issues this Deficiency Order. Within thirty (30) days of the date that this Order is filed, Plaintiff must either pay the $402 filing fee or submit a completed and signed in forma pauperis application that contains a statement of all the assets he possesses, states that he is unable to pay such fees, is accompanied by a notarized affidavit or declaration under penalty of perjury as discussed above, and is accompanied by a certified copy of Plaintiff's prison trust fund account statement (or institutional equivalent) for the prior six-month period.
