DEFICIENCY ORDER - Plaintiff is ORDERED to complete Page 7 of the Application form, sign it under penalty of perjury, and return it to this Court within 30 days. Plaintiff must list all civil cases and appeals that were dismissed as described in the statute quoted above. 28 U.S.C. 1915(g). Failure to disclose relevant dismissals may lead to sanctions. Alternatively, Plaintiff may pay $402 ($350 filing fee plus $52 administrative fee) within 30 days of the date of this Deficiency Order to proceed. The Clerk of Court is DIRECTED to provide a blank copy of Page 7 of the Application to proceed in forma pauperis to Plaintiff with this Order; ORDER TO SHOW CAUSE - Plaintiff is ORDERED to show cause why his claims should not be dismissed because they are barred by the two-year statute of limitations.
