September 25, 2019 Opinion or Order TEXT ORDER: Although Mr. Sevegny has submitted the affidavit required by 28 U.S.C. 1915(a)(1) in support of his Application to Proceed Without the Payment of Fees #2 , he has not submitted a certified copy of his inmate trust fund statement (or institutional equivalent) obtained from the appropriate official at the A.C.I., where he is currently incarcerated, as directed by 1915(a)(2). Therefore, the Court directs Mr. Sevegny to file a certified copy of his prisoner trust fund account statement (or institutional equivalent), obtained from and certified by an appropriate official at the A.C.I., within thirty (30) days of the date of this order. When the Court receives the certified copy, it will rule upon the Application. Failure to timely provide this required documentation will result in the motion being denied and the complaint dismissed. The Court notes that Plaintiff is still required to pay the statutory filing fee of $350 for this action.
